Our model of care is designed to provide comprehensive, personalised, coordinated, and patient-centred care for people with cancer and their families. Our aim is to support you throughout your entire care journey, from diagnosis through treatment and follow-up, ensuring high-quality care and continuous communication with your healthcare team.

We understand that every patient is unique. For this reason, we tailor our care to your clinical, emotional, and social needs, encouraging your active participation in decisions regarding your health.

How does our care model work?

Our organisation is based on a multidisciplinary model in which healthcare professionals work together in a coordinated manner to provide the best possible care.

Care is organised through specialised Functional Units according to tumour type. These units bring together professionals from different disciplines who jointly review each case and develop an individualised diagnostic, treatment, and follow-up plan.

This model enables us to:

  • Improve coordination between healthcare professionals.
  • Facilitate communication between patients and the healthcare team.
  • Ensure continuity of care.
  • Identify and manage complications or side effects at an early stage.
  • Provide comprehensive support throughout the entire care pathway.

Your healthcare team

During your care journey, you may be seen by a range of healthcare professionals, including:

  • Medical Oncologist: Responsible for coordinating your cancer treatment, monitoring your clinical progress, and leading the planning of your care.
  • Consultant Haematologist: Responsible for coordinating your haematological treatment, monitoring your clinical progress, and leading the planning of your care.
  • Clinical Oncology (Radiotherapy): Involved when radiotherapy forms part of your treatment plan.
  • Oncological Surgery: Provides surgical treatment when surgery is required as part of your care plan.
  • Pathology, Radiology, Nuclear Medicine and Genetics: Specialists involved in the diagnosis, characterisation, and monitoring of your disease.
  • Oncology and Haematology Nursing Team: Provides health education, symptom monitoring, treatment guidance, and ongoing support throughout your care journey.
  • Oncology and Haematology Day Unit: Administers treatments and provides clinical monitoring during therapy sessions.
  • Psycho-Oncology: Offers emotional and psychological support to patients and their families when needed.
  • Social Work: Helps identify social needs and facilitates access to community and health and social care resources.
  • Spanish Association Against Cancer (AECC): Provides support programmes, guidance, companionship, and resources for patients and carers.

When should you contact your healthcare team?

Please contact us if you experience:

  • Questions or concerns about your treatment.
  • Side effects related to your medication.
  • Persistent nausea, vomiting, or diarrhoea.
  • Uncontrolled pain.
  • Significant changes in your general condition.
  • Problems related to catheters or venous access devices.
  • The need for emotional or social support.

Attend the Emergency Department Immediately If You Experience:

  • A temperature of 38°C or above.
  • Difficulty breathing.
  • Chest pain.
  • Significant bleeding.
  • Neurological symptoms or sudden changes in neurological function.
  • Any other severe or sudden-onset symptom.
